- Includes all primary care services I can provide for you.
- Other services (labs, immunizations, X-rays, specialists, hospital costs...) are not included in this fee but may be covered by your insurance.

 All the same care as our basic package including...,
- $4,000 per year
- Includes all Primary care services I can provide for yo at your home at our mutual convenience, including monthly visits
- Other services (labs, immunizations, X-rays, specialists, hospital costs...) are not included in this fee but may be covered by your insurance.
